Admissions/Registration

At Calais Community Hospital our Registration department prides itself in providing our patients with excellent customer service. Our offices are located in the main lobby of the hospital. Please arrive at least 15 minutes ahead of your appointment time to register.  Please bring your insurance card(s) with you to the appointment so that we may verify your billing information.  If this is your first time visiting our hospital we will request photo identification.

The registration representative will also verify your phone number, address, employment information (if applicable), primary care provider, emergency contact information, and other non-clinical demographic information. 

If you have an insurance co-payment, co-insurance, or deductible you may be asked for a payment at the time of service. If you are a self-pay patient we will ask for a deposit for the services being rendered.

If you need help applying for a payment plan or financial assistance a registration representative can assist you with obtaining an application.

With every visit, you will be asked to sign a consent form for your medical treatment and release of information to your insurance company.

Once you have completed the registration process, the department where you will be receiving services will be notified that you have arrived and an employee will be out to greet you and escort you to your appointment. 

The Central Scheduling office can help with rescheduling appointments for outpatient services.   The department can be contacted at 207-454-9368 and is open Mon-Thu 8 am – 4:30 pm and Fri 8 am – 3 pm. 

Open Admissions Policy 

It is the policy of Calais Community Hospital, Calais, Maine, to admit and treat all persons without regard to race, color, sex, handicap, national origin, or religious creed. Admission requirements and assignments of hospital facilities are the same for all persons. There are no distinctions in eligibility for receiving any patient care services.
