The Case Management department at Calais Community Hospital consists of a Registered Nurse Manager and a Licensed Social Worker. The case management process is a multi-disciplinary program that works to assess, plan, coordinate, monitor and evaluate the options required to meet our patient’s healthcare needs. Effective communication and available resources are utilized to promote quality, cost-effective care.
The purpose of the department is to assist patients/residents and their families with healthcare issues and to develop monitor the process of discharge planning. Staff can make referrals to appropriate agencies in the community for support such as home health, assisted-living facilities, nursing facilities and other available community resources.
